Field level hazard assessments your crew will actually finish.
Open it on a phone at the work, walk through the hazards, sign it, and get on with the job. The office sees it right away, and it is already filed for your audit.
What a paper FLHA can’t do
A paper FLHA sits in a truck until somebody remembers to hand it in. By then the job is over, and nobody in the office could have seen a hazard the crew flagged that morning.
In Orientra the assessment is tied to the job and the site from the moment it starts. It comes with the hazard list, severity and likelihood scoring, the controls and the PPE, so a worker picks and fills rather than writes an essay on a phone in the cold.
Once it is signed it is locked. If you reword the form next spring, last winter’s FLHAs still read exactly as they did the day they were signed.
Built for the person holding the phone
- Works in a mechanical room or on a site with no signal, and sends when it can
- Only the requirements that apply to this job, not the whole safety program
- Your own wording and your own checks on top of the standard form
- A signature that is the signer’s own attested name
- You decide whether a missed FLHA reminds, asks for acknowledgement, or stops the job
- Workers do not need a paid login each: pricing is by the size of your workforce
Common questions
What is an FLHA?
A field level hazard assessment is the check a crew does at the work, before starting and whenever conditions change: what could hurt us here, how bad could it be, and what are we doing about it. Some companies call it an FLRA, a field level risk assessment. Orientra handles both.
What is the difference between an FLHA and a JHA?
A job hazard analysis looks at a task ahead of time and is written once, usually in the office. An FLHA is done on the day, by the people doing the work, for the conditions in front of them. A company needs both, and both live in Orientra.
Can we use our own FLHA form?
Yes. Start from the form that comes with Orientra or build your own, with your wording and your own checks. The hazard section stays, because a hazard assessment without one is not a hazard assessment.
What happens with no cell signal?
The form is saved on the phone and sent once the phone can reach Orientra again. The screen tells the worker what has been sent and what is still waiting, so nobody is told a record is safe before it is.
Who can see the completed FLHAs?
Supervisors and safety staff see them as they come in, tied to the job and the site. They also count toward your COR evidence without anybody filing them twice.
